TSA Beneficiary Nomination Form

The TSA Beneficiary Nomination Form allows you to update your beneficiary nominations for your unapproved group life cover & funeral cover through your employer group.

The original nomination form must be filed with your Human Resources department, and the most recent form will be used in the event of a claim.

It is also important to ensure that the beneficiaries nominated do not sign the form as witnesses, as this may prevent them from receiving the benefits.

Failing to provide a nomination form, the group life cover will be paid to your estate and distributed according to your will, or failing a valid will, according to intestate succession.
